REQUEST TO SUBMIT PROPOSAL FOR FOOD (4 DAYS), ACCOMMODATION (205 PAX FOR 2 NIGHTS), RENTALS OF TENTS (4 UNITS 8x12 & 2 UNITS 8x9) AND 10 INDUSTRIAL FANS (2 DAYS) DURING THE REGIONAL FARMERS’ FESTIVAL 2017 AT DA-RFO 5, SAN AGUSTIN, PILI, CAMARINES SUR AT APPROVED BUDGET FOR THE CONTRACT AMOUNTING TO EIGHT HUNDRED NINETY ONE THOUSAND FIVE HUNDRED PESOS (PHP891,500.00) PURSUANT TO ALTERNATIVE METHOD OF PROCUREMENT SPECIFICALLY NEGOTIATED PROCUREMENT - SMALL VALUE PROCUREMENT UNDER SECTION 53.9 UNDER RA 9184 AND ITS REVISED IRR

The Department of Agriculture - Regional Field Office No. V (DA-RFO V), San Agustin, Pili, Camarines Sur is inviting prospective suppliers to submit complete bid proposal for Food (4 Days), Accommodation (205 Pax for 2 Nights), Rentals of Tents (4 Units 8x12 and 2 Units 8x9) and 10 Industrial Fans (2 Days) during the Regional Farmers’ Festival 2017 at DA-RFO 5, San Agustin, Pili, Camarines Sur. The agency intends to apply the amount of Eight Hundred  Ninety One Thousand Five Hundred Pesos (Php891,500.00) as the Approved Budget for the Contract. Partial bid is allowed.

As per approved Purchase Request and  considering the nature of requisition being not more than One Million Pesos (Php1,000,000.00)  and in order to fast-track the implementation of the program, this procurement is facilitated thru Negotiated Procurement – Small Value Procurement under Section 53.9 of RA 9184 and its Revised IRR.

As such, ASOG GARDEN AND CATERING SERVICES, Naga City; TITO COOKS CATERING, Magarao, Camarines Sur; CHEF MARCIANO’S KITCHEN, Naga City; VILLA CACERES HOTEL, CBD PLAZA HOTEL and EUROTEL, all in Naga City; BRENCE P. CATERING SERVICES, Naga City; CAMILLE CATERING SERVICES, Palestina, Pili, Camarines Sur, are hereby requested to submit the following documents:   Income/Business Tax Return

  1. 2017 Mayor’s Permit
  2. PhilGEPS Registration
  3. Omnibus Sworn Statement

The bidders are required to pay Php500.00 for bid documents to the Cashier’s Office, DA-RFO V, San Agustin, Pili, Camarines Sur.  All submitted documents must be in duplicate, otherwise, bids will be automatically rejected on the ground of non-compliance with the prescribed bid requirements.

Sealed Bid must be received by BAC Secretariat, DA-RFO V not later than 1:30 in the afternoon of 24 April 2017 at Conference Room, New Admin. Building, DA-RFO V, San Agustin, Pili, Camarines Sur. Opening of proposal will be on the same date and time.
