              The Department of Agriculture Regional Field Unit No. 5 (DA RFO 5), San Agustin, Pili, Camarines Sur is inviting interested caterers to submit complete bid proposal for Food & Accommodation for the conduct of Special Area for Agricultural Development (SAAD) Provincial Orientation and Monitoring of Yolanda Rehabilitation and Recovery Program in:

 The agency intends to apply the amount of Two Hundred Fifty Two Thousand Pesos (Php252,000.00) as the Approved Budget for the Contract.

            As per approved Purchase Request and  considering the nature of requisition being not more than One Million Pesos (Php1,000,000.00)  and in order to fast-track the implementation of the program, this procurement is facilitated thru Negotiated Procurement-Small Value Procurement under Section 53.9 of RA 9184 and its Revised IRR. PARTIAL BID IS ALLOWED.

As such, (LOT 1) – RHAJ INN, Virac, Catanduanes; RAKDELL INN, Virac, Catanduanes; and MIDTOWN INN, Virac, Catanduanes

As such, (LOT 2) – PAUL ANGELO CATERING SERVICES, Sorsogon City; MANGO GRILL, Sorsogon City and FERNANDO’S HOTEL, Sorsogon City; and

As such, (LOT 3) – UNICA HIJA HOTEL & RESTO, Masbate; FRESHCO BEACH RESORT, Masbate City; and WINTER’S FARM RESORT, Milagros, Masbate;

Are hereby requested to submit the following documents:

  2. Bid Proposal
  3. Mayor’s Permit
  4. PhilGEPS Registration
  5. Income Tax Return
  6. Omnibus Sworn Statement
  7. BIR Registration

The bidders are required to pay Php500.00 for bid documents to the Cashier’s Office, DA RFO-5, Pili, Camarines Sur.  All submitted documents must be in duplicate, otherwise, bids will be automatically rejected on the ground of non-compliance with the prescribed bid requirements.

Sealed Bid must be received by BAC Secretariat, DA RFO 5 not later than 1:30 in the afternoon of 16 October 2017 at DA Library, DA RFO-V, Camarines Sur. Opening of quotation will be on the same date and time.
