REQUEST TO SUBMIT QUOTATION FOR LABOR, MATERIALS AND INSTALLATION OF AUDIO BOOTH FOR USE AT TRAINING HALL LOCATED AT 3RD FLOOR OPERATIONS BUILDING AT APPROVED BUDGET FOR THE CONTRACT AMOUNTING TO ONE HUNDRED FIFTY NINE THOUSAND NINE HUNDRED THIRTY ONE PESOS AND SIXTY SEVEN CENTAVOS(PHP159,931.67) PURSUANT TO ALTERNATIVE METHOD OF PROCUREMENT SPECIFICALLY NEGOTIATED PROCUREMENT-SHOPPING UNDER SECTION 52.1 UNDER RA 9184 AND ITS REVISED IRR.

The Department of Agriculture Regional Field Unit No. 5 (DA RFO 5), San Agustin, Pili, Camarines Sur is inviting interested suppliers to submit complete bid quotation for Labor, Materials and installation of Audio Booth for use at Training Hall Located at 3rd Floor Operations Building. The agency intends to apply the amount of One Hundred Fifty Nine Thousand Nine Hundred Thirty One Pesos and Sixty Seven Centavos (Php 159,931.67) as the Approved Budget for the Contract.

 As per approved Purchase Request and  considering the nature of requisition being not more than Two Hundred Thousand Pesos (Php200,000.00)  and in order to fast-track the implementation of the program, this procurement is facilitated thru Negotiated Procurement-Shopping under Section 52.1 of RA 9184 and its Revised IRR.

As such, MTJ CONSTRUCTION AND SUPPLY, J. Dudya St, Brgy 4 Poblacion, Camalig, Albay; HDB CONSTRUCTION, 8th Street, Villa Sorabella Subdivision, Concepcion Grande, Naga City ; MAGDA CONSTRUCTION AND SUPPPLY, PNR Road, Purok 4, Salvacion, Daraga City, and any other interested/qualified suppliers are hereby requested to submit the following documents:

  1. PhilGEPS Registration
  2. Mayor’s Permit
  3. BIR Registration

The bidders are required to pay Php500.00 for bid documents to the Cashier’s Office, DA RFO-5, Pili, Camarines Sur.  All submitted documents must be in duplicate, otherwise, bids will be automatically rejected on the ground of non-compliance with the prescribed bid requirements.

Sealed Bid must be received by BAC Secretariat, DA RFO 5 not later than 1:30 in the Afternoon of  29 October 2018 at DA Library, Research Building, DA RFO-5, Pili, Camarines Sur. Opening of quotation will be on the same date and time.
