REQUEST TO SUBMIT QUOTATION FOR MEALS AND ACCOMMODATION DURING THE CONDUCT OF RPCO-PPMIU 1ST SEMESTER ASSESSMENT & COORDINATION MEETING ON JUNE 7, 2018 IN NAGA CITY UNDER 2018 PHILIPPINE RURAL DEVELOPMENT PROGRAM (PRDP) AT ESTIMATED PROJECT COST AMOUNTING TO ONE HUNDRED THREE THOUSAND SEVEN HUNDRED FIFTY PESOS (PHP103,750.00)

The Philippine Rural Development Project thru the Department of Agriculture Regional Field Office No. 5 (DA RFO 5), San Agustin, Pili, Camarines Sur is inviting interested suppliers/dealers to submit Quotation for Meals and Accommodation during the conduct of RPCO-PPMIU 1st Semester Assessment & Coordination Meeting on June 7, 2018 in Naga City. The Project intends to apply the amount of One Hundred Three Thousand Seven Hundred Fifty Pesos (Php103, 750.00) as the Estimated Project Cost.

As such, VILLA CACERES HOTEL & RESORT, Magsaysay Avenue, Naga City; STARVIEW HOTEL, Elias Angeles St., Naga City and REGENT HOTEL, Naga City, are hereby requested to submit quotation with the following requirements and payment of P500 for bid documents:

  1. Philgeps Registration (Platinum)
  2. Mayor’s Permit
  3. DTI or SEC Registration
  4. Tax Clearance Certificate (for bidding purposes)
  5. Omnibus Sworn Statement
  6. Bid Proposal
  7. Menu

 

Sealed Bid must be received by BAC Secretariat, DA RFO 5 not later than 1:30 in the afternoon of 28 May 2018 at DA Library, DA RFO-V, Camarines Sur. Opening of quotation will be on the same date and time. All submitted documents must be in duplicate otherwise, bids will be automatically rejected on the ground of non-compliance with prescribed bid requirement.
