NOTICE TO CONDUCT REPEAT ORDER

June 21, 2018

The Department of Agriculture Regional Office No.5 through BAC Resolution No. 1600-930 is conducting a Repeat Order for the Procurement of 4,500 bags Urea Fertilizer, 4,500 bags Complete Fertilizer and 1,500 bags Ammonium Phosphate under 2018 Rice Program that was awarded to La Filipina Uy Gongco Corporation with office Address at Quarantine Road, Brgy. Progreso, Lapuz, Iloilo City.  The Contract Agreement between The Department of Agriculture Regional Office No. 5 and La Filipina Uy Gongco Corp. was executed on April 1, 2018. The Notice to Proceed (NTP) was made on April 10, 2018: The original amount of the contract was Forty Six Million Four Hundred Seventy Six Thousand Pesos (Php46, 476,000.00).

The Repeat Order shall be:

Bid must be received by BAC Secretariat not later than 9:30 in the morning of June 27, 2018 at DA RFO-V, San Agustin, Pili, Camarines Sur. Opening of proposal will be on the same date and time.

The repeat order for the previous contract shall be governed by the RA 9184 and its Implementing rules and Regulations.

The Department of Agriculture, Region 5 shall in no case be held responsible or liable for any and all costs associated in the implementation of the Repeat Order of its result
