REQUEST TO SUBMIT QUOTATION FOR THE SUPPLY AND DELIVERY OF (LOT 1) OFFICE SUPPLIES, (LOT 2) AGRICULTURAL SUPPLIES AND (LOT 3) VARIOUS/OTHER SUPPLIES FOR USE AT DA-ARDC (AES), MAYON CROSSING, TABACO CITY AT APPROVED BUDGET FOR THE CONTRACT AMOUNTING TO ONE HUNDRED TWENTY SIX THOUSAND PESOS AND 50/100 (PHP126,000.50) PURSUANT TO ALTERNATIVE METHOD OF PROCUREMENT SPECIFICALLY NEGOTIATED PROCUREMENT – SHOPPING UNDER SECTION 52.1 UNDER RA 9184 AND ITS REVISED IRR.
The Department of Agriculture Regional Field Office No. 5 (DA RFO 5), San Agustin, Pili, Camarines Sur is inviting interested suppliers to submit complete quotation for the supply and delivery of:

As per approved Purchase Request and considering the nature of requisition being not more than Two Hundred Thousand Pesos (Php200,000.00) and in order to fast-track the implementation of the program, this procurement is facilitated thru Negotiated Procurement-Shopping under Section 52.1 of RA 9184 and its Revised IRR.

As such, (LOT 2) – PACIFICA AGRIVET SUPPLIES, INC, Legazpi City, Albay; SANONE DOROTHY SUPPLY, Ligao City, Albay; and MARIA CORAZON D. GLORIOSO HARDWARE, Ligao City, Albay;
As such, (LOT 3) – MARIA CORAZON D. GLORIOSO HARDWARE, Ligao City, ALbay; SZEMAN ENTERPRISES, Ligao City, Albay; and BARCELONA TRADING, Ligao City, Albay are hereby requested to submit the following documents:
- Mayor’s Permit
- PhilGEPS Registration
- BIR Registration
The bidders are required to pay the non-refundable amount Php500.00 for bid documents to the Cashier’s Office, DA RFO-5, Pili, Camarines Sur. All submitted documents must be in duplicate, otherwise, bids will be automatically rejected on the ground of non-compliance with the prescribed bid requirements.
Sealed quotation must be received by BAC Secretariat, DA RFO 5 not later than 1:30 in the afternoon of 23 July 2018 at DA Library, DA RFO 5, Camarines Sur. Opening of quotation will be on the same date and time.
