REQUEST TO SUBMIT PROPOSAL FOR THE FOOD & ACCOMMODATION AND TRAINING KIT FOR USE DURING THE ASPIRE FAIR & EXHIBITS AND ENTREPRENEURIAL CAPABILITY SEMINAR DURING THE BICOL BUSINESS MONTH (BBM) AT NAGA CITY ON JULY 18-21, 2018 AT APPROVED BUDGET FOR THE CONTRACT AMOUNTING TO FIVE HUNDRED TWO THOUSAND FIVE HUNDRED PESOS ONLY (PHP502,500.00) PURSUANT TO ALTERNATIVE METHOD OF PROCUREMENT SPECIFICALLY SMALL VALUE PROCUREMENT UNDER SECTION 53.9 UNDER RA 9184 AND ITS REVISED IRR

The Department of Agriculture Regional Field Unit No. 5 (DA RFO 5), San Agustin, Pili, Camarines Sur is inviting interested suppliers to submit complete bid proposal for the ff:

The agency intends to apply the amount of Five Hundred Two Thousand Five Hundred Pesos Only (PHP502,500.00) as the Approved Budget for the Contract. Partial bid is not allowed.

As per approved Purchase Request and considering the nature of requisition being not more than One Million Pesos (Php1,000,000.00) and in order to fast-track the implementation of the program, this procurement is facilitated thru Negotiated Procurement-Small Value Procurement under Section 53.9 of RA 9184 and its Revised IRR.

As such, Lot 1: ROBRIEDO INC. (VILLA CACERES HOTEL), Naga City; AVENUE PLAZA HOTEL, Naga City and STARVIEW HOTEL, Naga City;

Lot 2: SANVIC CATERING SERVICES, Canaman, Camarines Sur; COCINA NI MIGAY, Naga City and BODEN CATERING SERVICES, Naga City

Lot 3: BONING’S TRADING, Naga City; LUCKY EDUCATIONAL SUPPLY INC. and PAPEL AT PLUMA, Naga City are hereby requested to submit the following documents:

  1. Mayor’s Permit
  2. DTI Registration/SEC. Registration
  3. PhilGEPS Registration
  4. Annual Income Tax Return
  5. Omnibus Sworn Statement
  6. BIR Registration
  7. Menu

The bidders are required to pay Php500.00 for bid documents to the Cashier’s Office, DA RFO-5, Pili, Camarines Sur.  All submitted documents must be in duplicate, otherwise, bids will be automatically rejected on the ground of non-compliance with the prescribed bid requirements.

Sealed Bid must be received by BAC Secretariat, DA RFO 5 not later than 9:30 in the morning of 11 July 2018 at DA Library, DA RFO-V, Pili, Camarines Sur. Opening of proposal will be on the same date and time.
