NOTICE TO CONDUCT REPEAT ORDER
June 19, 2018

The Department of Agriculture Regional Office No.5 through BAC Resolution No. 1598-928 is conducting a Repeat order for the Procurement of Five Hundred Twenty Eight (528)bags Vermicast Fertilizer under 2018 Organic Agriculture Program that was awarded to RNM Agro./Industrial Supply  and Gen. Mdse with office Address at Zabala St., Brgy. V, Daet, Camarines Norte.  The Contract Agreement between The Department of Agriculture Regional Office No. 5 and RNM Agro./Industrial Supply  and Gen. Mdse with office Address at Zabala St., Brgy. V, Daet, Camarines Norte was executed on April 18, 2018. The Notice to Proceed (NTP) was made on April 23, 2018: The original amount of the contract was Two Million Three Hundred Twenty Nine Thousand Nine Hundred Ninety-Eight Pesos (Php2,329,998.00).

The Repeat Order shall be:

Bid must be received by BAC Secretariat not later than 9:30 in the morning of June 27, 2018 at DA RFO-V, San Agustin, Pili, Camarines Sur. Opening of proposal will be on the same date and time.

The repeat order for the previous contract shall be governed by the RA 9184 and its Implementing rules and Regulations.

The Department of Agriculture, Region 5 shall in no case be held responsible or liable for any and all costs associated in the implementation of the Repeat Order of its result.
