SECOND REQUEST TO SUBMIT PROPOSAL FOR THE SUPPLY AND DELIVERY OF THE FOLLOWING: LOT 1 – 500 kgs GINGER; LOT 2- 2,700 pcs. BUDDED CALAMANSI & LOT 3 – 10 rolls PLASTIC MULCH & 200 bags ORGANIC FERTILIZERS  FOR DISTRIBULTION TO FARMEERS FOR GINGER, CITRUS & VEGETABLE PRODUCTION  UNDER 2020 HVCDP AT APPROVED BUDGET FOR THE CONTRACT AMOUNTING TO THREE HUNDRED FIFTY THOUSAND PESOS (PHP 350,000.00) PURSUANT TO ALTERNATIVE METHOD OF PROCUREMENT SPECIFICALLY SMALL VALUE PROCUREMENT UNDER SECTION 53.9 UNDER RA 9184 AND ITS REVISED IRR

                The Department of Agriculture Regional Field Office No. 5 (DA RFO 5), San Agustin, Pili, Camarines Sur is inviting interested suppliers to submit complete quotation for the  supply and delivery of  Lot 1 – 500 kgs. GINGER; Lot 2 – 2,700 pieces BUDDED CALAMANSI & Lot 3 – 10 rolls PLASTIC MULCH &  200 bags ORGANIC FERTILIZER for distribution to farmers for ginger, citrus & vegetable production under   2020 HVCDP. The agency intends to apply the amount of  THREE HUNDRED FIFTY THOUSAND PESOS  (PHP 350,000.00) as the Approved Budget for the Contract.

            As per approved Purchase Request and considering the nature of requisition being not more than One Million Pesos (Php1,000,000.00) and in order to fast-track the implementation of the program, this 4procurement is facilitated thru Negotiated Procurement-Small Value Procurement under Section 53.9 of RA 9184 and its Revised IRR.

            As such: GMG AGRI-FARM PRODUCTS;  GREENSEEDS GARDEN & PLANTERS; BERDIES AGRI-TRADING & GREENWORLD AGRI-FARM CENTER and All Interested Suppliers/Bidders are required to submit the following documents:

  1. Mayor’s Permit
  2. PhilGEPS Registration
  3. DTI/SEC. Registration
  4. BIR Registration
  5. Annual Income Tax Return
  6. Omnibus Sworn Statement
  7. Certificate of authority from Source ( for ginger & Budded Calamansi)
  8. BAFS Product Registration (for Organic fertilizer)
  9. OCCP Certification (for organic Fertilizer)

            The bidders are required to pay the non-refundable amount Php500.00 for bid documents to the Cashier’s Office, DA RFO-5, Pili, Camarines Sur.  All submitted documents must be in duplicate and marked with index/ear tabs or side end tabs to identify the page components, otherwise, bids will be automatically rejected on the ground of non-compliance with the prescribed bid requirements.

            Sealed quotation must be received by BAC Secretariat, DA RFO 5 not later than 9:30 in the morning of  21 September  2020  at DA-BAC Office, DA RFO 5, Camarines Sur. Opening of quotation will be on the same date and time.
